Bulgarian to English Medical Medical: Health Care Medical questionnaire
This is for a back translation of a medical questionnaire where the original was in English, so there's always the possibility that the Bulgarian translation is incorrect. The context for the term is as follows:

Въведете стойността от колоната с точки в съответното поле от колоната „Не зная“ за този въпрос.

My guess is that “колоната с точки” should be translated as “points column,” rather than “dots column.” However, if anyone has a better idea, I would love to hear about it.

I have also often seen "score" for "точки" in medical questionnaires. Impossible to guess which one is most suitable in a back translation though

The idea is definitely about something graded, such as answers from 1 to 5, or 1 to 10.

Indeed, it is 'points column'. In a questionnaires things quite often are ranked/evaluated by points or by other means.

grade column

I would rather say "grade column" because certain things in questionnaires are determined by grades or degrees of intensity, e.g. pain. I could not find a sample document, however.
