Assimina, Please ignore the 'reference number' part of this answer. I have found adequate Internet references to confirm/support the suitability of 'code number', and so I would prefer to stick with 'code number' only. (I also write this as 'code number' on every Greek criminal record that I translate).
Peer comment(s):

agree Spyros Salimpas : I agree only with the first term, as the latter is typically used for the greek term "αριθμός πρωτοκόλλου".

Thank you, and I agree with you, but I would have had to remove my answer to delete the 'reference number' part.
agree rokotas : It also occured to me recently, and since I could not find any specialized translation, I wrote: 'code number'. Besides I agree with Spyros about 'reference'.

Thank you. See my notes to Assimina and Spyros regarding 'reference'. I always translate 'κωδικάριθμος' as 'code number' when I translate criminal records.
agree Vasilisso : code number
