I am trying to establish whether this should be translated simply as 'coal' or whether it ALWAYS refers to coal with a particularly high carbon content. i.e. hard coal (or possibly black coal).

There are many types of coal: anthracite, gas coal, cannel coal, durain, fat, coking, lump, steam-baking, semi-bright, sapropelic, etc., etc. When one refers to "coal" one generally means "hard coal" but in the given case perhaps the distinction is needed

I think they rather differ it by quality. Black coal as opposite to brown one has more dense and crystalline structure and higher heat of combustion. In this sense "bituminous coal" will suit too.

Is каменный уголь contrasted with any other kind of уголь in your text? Judging by some sources, there are just two main kinds, каменный and бурый, which would usually be called simply 'coal' and 'lignite'.

there are kinds of coal, like lignite (brown coal) which are definitely not "kamennyi ugol)
