"along the road" is optional and you can omit it if you wish.

In the U.K. we would not understand "Km 20" by itself because we do not use these kilometre point markers in the addresses of some buildings, as they do in Spain.

around the 20 km mark

This "el Km 20" identifies a specific marker ("milestone", in its strict sense, were miles in use). It is a spot on the highway, which happens to be located at a distance of 20 km from some established starting point (Km 0). O the other hand, "a la altura" allows for a certain imprecision, hence "around"
